Applying for and preparing yourself for an Autism & ADHD Assessment

If you are considering an autism or ADHD assessment for yourself, or your child, it can be hard to know where to start. Completing Autism and ADHD referral paperwork requires gathering detailed, evidence-based examples of behaviours from home and school/work settings, involving parents/carers sometimes friends and family as well as staff in alternative recreational settings, often using specific questionnaires like AQ-10 (Autism) and ASRS (ADHD).

If you think you might be autistic or ADHD, an assessment can be a validating experience and give you an understanding of yourself and the confidence to support any changes in your life if needed. To ask for a referral for an autism or ADHD assessment, it will vary depending on age and your geographical location. Some NHS trusts accept referrals directly from a medical professional, and others will only accept them from schools. As mentioned, they require the completion of paperwork and specific forms that act as a triage to decide if you meet the criteria for an assessment. You also have the “Right to Choose” offer, which again varies depending on where you live.

Feedback I receive is that the difficult part is understanding what the question is asking and associating it to relevant examples. Many parents will tell me that it is challenging to know what the forms are wanting to know, and after sometimes years of waiting, they feel pressured that they have missed out so much relevant and important information that may impact a decision. For adults that have made the decision to request an assessment, the thought of completing a set of referral forms and the mental and physical effort required can be extremely daunting and overwhelming. Having supported lots of parents and adults in completing this paperwork, I have the knowledge, skills and experience to explain the questions and rephrase them in a neurodiverse friendly way with context to allow the time and space to process what information is needed.

  • Complete Questionnaires: For adults, this usually involves self-report forms (ASRS for ADHD, AQ-10 for autism).
  • Child Referrals: These often require input from both parents/carers and school staff to build a full picture.

Information Gathering

As a neurodiverse individual I understand how it can be hard to remember certain events in the past and it is through careful questioning that this information comes to light. It is sometimes useful to look back over old photos or videos to remind yourself of experiences and talking about things you or your child used to do. These discussions will help to prompt memories to gather a portfolio of information to give examples in the paperwork and for you to take to your appointment.

The paperwork and assessment will want to know the details in the form of examples which tell the assessors about frequency, duration and the impact on you or your child’s life. For example, how often it occurs, how long this lasts for and the impact of how you or they feel about this. An additional part of my support is gathering child voice through a range of therapeutic activities to ensure that they are heard, and this is conveyed to the assessors.
